As per the official coding guidelines Codes in category 338 may be used in conjunction with codes from other categories and chapters to provide more detail about acute or chronic pain and neoplasm-related pain, If Pain is mentioned as Acute, then assign 338 category code and pain code: Example-Acute Low Back Pain, assign the 338 category code and Low back pain code, 724.2 If pain is not mentioned as acute or chronic, then dont assign 338 code When admission is for Pain management, then assign 338 as first listed code and then underlying cause. Example- Pain due to inter-vertebral disc- Code first 338 and then disc proplapse. When the admission is for procedure, then 338 as secondary code If pain is due to device or implants and grafts, code first complication code and then 338 Postoperative pain may be reported as the principal or first-listed diagnosis when the stated reason for the admission/encounter is documented as postoperative pain control/management. Chronic pain is classified to subcategory 338.2. There is no time frame defining when pain becomes chronic pain. The provider’s documentation should be used to guide use of these codes. When the reason for the admission/encounter is management of the neoplasm and the pain associated with the neoplasm is also documented, code 338.3 may be assigned as an additional diagnosis. http://www.medesunglobal.com
