Anaphylaxis is defined as an acute multi-system severe type 1 hypersensitivity allergic reaction and typically, between one and 15 per cent of western countries’ populations are "at risk" of an anaphylactic reaction when exposed to allergens. As many as one percent of sufferers may die as a result. Anaphylaxis has no universally accepted clinical definition. It is a clinical diagnosis based on typical systemic manifestations, often with a history of acute exposure to a causative agent. Because anaphylaxis is primarily a clinical diagnosis, laboratory studies are not usually required and are rarely helpful. However, if the diagnosis is unclear, especially with a recurrent syndrome, or if other diseases need to be excluded, some limited laboratory studies are indicated. Skin testing and in vitro IgE tests may be helpful. The greatest success in preventing anaphylaxis has been the use of allergy injections to prevent recurrence of sting allergies. Unfortunately, no two victims can be guaranteed to react in the same way. Alsco First Aiders learn that the risk to each individual from a particular species of insect depends on the person bitten or stung, insect aggression, the efficiency of the venom delivery apparatus and the venom’s strength and likelihood of causing an allergic reaction. However, insect stings are not the only high risk area for employees and site visitors, in a small percentage of the population, anaphylaxis also can occur as a result of exposure to peanuts or other nuts. (Some treatments for nut allergies are in development, but approved release is as yet some way off.) Generally speaking, anaphylaxis may require resuscitation measures such as airway management, oxygen, and intravenous fluids and close monitoring. Administration of epinephrine is the ‘treatment of choice’ with antihistamines and steroids often also used. Hospital observation for between six and 24 hours is recommended for victims even after they have ‘returned to normal’.
