As a fetus grows, different body parts take different times to grow. In all these stages, the last body part to grow is the lungs. Fetal maturity of the lungs in different babies will happen at different rates. As the fetus grows, its ability to adapt to the extra-uterine process will depend on whether its pulmonary system is functioning well. An infant born with difficulty in respiration is termed to have respiratory distress syndrome- RDS. This is mostly brought about by insufficient levels of surfactants in its lungs. The lack of proper levels of surfactants is primarily considered the main etiological defect leading to RDS. In fetal maturity, surfactants are very important. Surfactants are surface tension limiting substances that play a vital role in the overall proper growth of the lung. They are a complex blend of proteins, carbohydrates and lipids and have three primary roles in the body of the developing fetus. Firstly, they help in reducing surface tension ensuring there is lesser pressure aiding in keeping alveoli open. Surfactants also stabilize the alveolar size by varying the surface tension. The substance also deters the exudation of liquids into the infant’s airways from the pulmonary circulation. Fetal Maturity - A mature lung contains about 95% phospholipid lipids; the main and most active component being lecithin (dipalmitoyl lecithin). Phosphatidylglycerol (PG) comes in second with others like phosphatidylethanolamine (PE), phosphatidylinositol (PI), lysolecithin, sphingomyelin and phosphatidylserine (PS) playing an important role in overall fetal maturity. To estimate the rate and level at which fetal maturity is taking place, the (L/S) lecithin to sphingomyelin ratio in the amniotic fluid has to be determined. Earlier, the two-dimensional chromatographic thin layer procedure, Gluck, was used to determine fetal maturity of the lung in fetuses. Today, Helena labs, Helena Fetal-Tek 200 one-dimensional procedure is providing faster and better means of estimating this.By using Fetal-Tek 200, phospholipids like sphingomyelin and lecithin are extracted using a chloroform-methanol mix and streaked on a thin layered chromatography plate. Helena labs Fetal-Tek 200 fetal maturity procedure offers a marker that helps in identifying these phospholipids.
