Slip disc or Spinal disc herniation is a condition where the outer fibrous ring of the intervertebral disc sustains a tear allowing the soft central tissues to bulge out. Symptoms of a herniated disc can vary depending on the location of the herniation and the types of soft tissue that become involved SIGNS AND SYMPTOMS • Negligible injury in the tissue may lead to little pain or sometimes no pain. • If the bulged herniations compresses the nerve roots, the pain may be appreciated through the course of nerve due to irritatation. • undefined pains in the thighs, knees or feet. • numbness, tingling, muscular weakness, paralysis, and affection of reflexes • If the herniated disc is in the lumbar region the patient may also experience, pain usually continuous or at least is continuous in a specific position of the body. CAUSES • Disc herniations can result from general wear and tear, such as when performing jobs that require constant sitting. • Herniations can result from jobs demanding lifting of heavy weights. • Traumatic injury to lumbar discs commonly occurs when lifting and bending at the waist, DISC PROLAPSE AND SCIATICA Sciatica is a pain in the leg caused by compression and/or irritation of one of five nerve roots that are branches of the sciatic nerve.there are several cuases for Sciatica like pregnancy, degeneration etc.. one being disc herniation itself. In Ayurveda Singapore the conditions which can result in backpain include external traumatic factors (abhighataja), and internal factors like doshas. We can co relate the symptoms of the sciatica with the condition called as ‘Gridhrasi’, again a Vatavyadhi. The disease is so called as the gait of the person alters due to pain and starts walking like a Vulture (Gridra). NIDANA There is no specific Nidana mentioned but for general Vatavyadhinidana. But considering the injury of tissues which leads to this condition we can assume abhighata as one of the nidana, included as one of ViharajaVataNidana. ROOPA The main symptom of the disease is radiating pain which starts from Sphikpradesha (buttocks0 to kati, prishta, uru, janu, jangha and paada respectively. Apart from these symtomsAcharyaSushrutha also mention SaktiUtkshepaNigraha as one of the symptom which is exactly the Sign called Straight Leg Raising (SLR). SAMPRAPTI Gridhrasi being a Vatavyadhi involves two types of samprapti which are Dhatukshayajanya and Margavarodha. Dhatukshayajasamprapti can be seen in degeneration of the vertebral bones resulting in the reduced disc space causing compression of the underlying nerves, thus causing the pain or set of symptoms. Margavarodha type of samprapti can be understood in vatakaphaja type of Gridhrasi where the vata is obstructed due to prakupitakaphadosha in katipradesha causing the symptoms along with aruchi and gouravata. Abhighata or trauma is a exogenous cause of disease. Here due to lifting of heavy weights or pregnancy causes the Gridhrasi, due to which the vatadosha gets vitiated and lodges in the katipradesha. Vata being the root cause of pain, the pain starts in the katipradesha and also involves the sira and khandaras of the lower extremities ( typically either side). LINE OF TREATMENT We can treat in the general line of snehana, swedana, vamana, virechana, niruhabasti, anuvasanabasti, agnikarma, siravyadha, and Slip Disc in Singapore shastrakarma.
