The use of donor eggs -- whether from family, friends or strangers -- by infertile patients undergoing in vitro fertilization (IVF) has risen in the last decade, and results have been favorable, with an increasing number of healthy babies born at term.Today women are increasingly using donated eggs to get pregnant, with often good results.Because the donor eggs are from young, healthy women, they have a good chance of success, generally regardless of the recipient's age.

Inspite of high success rate of assisted reproduction methods all couples are not able to achieve pregnancy by using their own eggs. These couples have only two options to have the family - receive a donated egg or sperm or get through a tedious process of adoption.

The Donor Egg Program was established to help women who are unable to use their own eggs (oocytes) to achieve pregnancy. A Donor Egg IVF cycle is an IVF cycle in which the eggs from a Donor are used instead of a patient's own eggs. Donor eggs may be combined with the male partner's sperm or with Donor sperm. Donor egg IVF is an appropriate treatment option for women with decreased ovarian reserve because of advanced reproductive age, premature ovarian failure and surgical menopause. It is also suitable for women carrying a genetic mutation and for those with multiple failed IVF cycles using own eggs. Poor egg quality is sometimes determined during an IVF attempt.

Donor Egg IVF opens up a world of possibilities and new hope for couples who have struggled to concieve. Once your pregnancy is established, your body takes over and there is nothing that can distinguish a Donor Egg pregnancy from a natural pregnancy. You have the oppurtunity to carry and give birth to your child.

Here are the most frequently asked questions about donor egg ivf cycle.

1. For whom is the donor egg advisable?

There are two primary indications for Egg Donation Treatment. They are age and ovarian reserve. As a woman increases in age, her eggs age as well. Women will experience a significant decline in fertility starting in their mid 30’s. The decline becomes more pronounced as a woman reaches her 40s with a near zero pregnancy rate utilizing her own eggs by age 45. Decreased ovarian function can occur in younger women and in severe cases complete ovarian failure (menopause) can occur in women under the age of 40. This decline is more rapid after the age of 37 and is seen in the quality as well as quantity of eggs produced.

2. Who are the Egg Donors ?

Most of the time egg donation process is anonymous and confidential. The identity of the egg donor and the egg recipient is not revealed unless otherwise stipulated. The Egg Donors are between the ages of 21 to 29, usually having atleast one child of their own and have undergone a complete psychological and medical evaluation and are screened for infectious diseases like HIV and hepatitis B. The intelligence level, height, weight and looks of the donor are also considered. Only after the screeing process is complete, the donor is recruited into the egg donor program.

3. What does the Donor Egg IVF procedure involve ?

Recipient couples are asked to select one or more donors whom they find suitable. One of the selected egg donors will be offered to a recipient couple upon availability of the egg donor. Final approval of the specific selection is always made by the recipient couple. The cycles of the donor and recipient to accomplish fresh embryo transfer whenever possible is co ordinated. Synchronization of cycles includes using a series of medications to facilitate a hospitable uterine environment for transfer of embryos. Once the Donors eggs are mature, usually in 11-12 days, the egg retrieval procedure will be done under anaesthesia using ultrasound guidance. The husband will provide his semen sample and fertilization will be carried out in the laboratory. Two to five days later 2-5 embryos will be transferred with great care into the Recipient's uterus. If there are embryos in excess of the number for safe transfer, Vitrification of additional embryos is available and is strongly recommended. Vitrified embryos (frozen embryos) can be used for subsequent attempts at pregnancy whether or not the fresh transfer is successful, and still produce high rates of success.

4. Is there a waiting list to start Donor Egg IVF treatment ?

Normally donor egg recipient is not required to join a waiting list. Donor Egg IVF cycles can match one donor to one recipient (sole match) or one donor to two recipients (shared match). Regardless of whether a patient chooses a sole or shared match, she can begin treatment immediately and will not be required to wait for a donor. An extensive pool of fully-screened, immediately available egg donors and list of frozen donor eggs is available with the clinic. The only waiting period is the duration required to synchronise the Egg Donor's menstrual cycle with the Recepients.

5. How can I be a Donor Egg Recepient if I live out of town or international patient?

Patients from overseas or abroad need to be with IVF specialist for three to five days from the day of the Donor's egg retrieval and can travel back home immediately after the Embryo Transfer. The initial coordination and evaluation of test results can be done through Email corresspondence. Your partner can come with you directly on the day of the Donors Egg retrieval and provide his Semen sample. Alternatively, he can travel anytime beforehand and freeze his semen sample at the IVF clinic.

7. What are the legal aspects of egg donation?

The ICMR guidelines allow commercial egg donation, provided it is anonymous and confidential. The egg donor has no rights over her eggs once they leave her body, and signs a contract agreeing to this.

8. Do I need to divulge to anyone that I used donated eggs?

It is your decision if, and to whom, you choose to share the genetic source of your child. It is strongly recommended that you do share the information with your Ob/Gyn and ultimately with your child’s pediatrician, so they can better take care for both you and your child. It is also advisable to share the information with your child once he grows up.
