Changes in Endocrinal secretions levels and Heredity are believed to be the factors responsible for occurrence of tumors. When the estrogen levels rise, fibroids swell. They can occur in the midst of pregnancy when estrogen levels are elevated. Risk of developing these benign growths is low for women having menopause because their estrogen levels are reduced. Fibroids are classified on the basis of where they occur. Types:- • Intramural:- They are most common type of fibroids located in the uterine wall. • Subserosal:- They develop outside the wall of uterus. They can become quite large. • Sub mucosal:- They are located in the muscle beneath the lining of the uterus wall. • Cervical:- They are located in the cervix. Symptoms:- Symptoms might not be apparent in some women. • Heavy periods • Anemia • Backache • Constipation • Abdominal pain • Swelling in the lower abdomen • Frequent urination • Painful sex Complications:- • Menorrhagia (heavy periods) • Infertility • Labor problems • Premature birth/Repeated Miscarriages • Leiomyosarcoma(cancer) Diagnosis:- • Vaginal Examination • Ultrasound • Trans-vaginal scan- scanner is inserted into vagina for closer view of uterus. • Hysteroscopy-small telescope inserted in womb • Laparoscopy- to examine Subserosal tumors • Biopsy- examination of lining of uterus wall Treatment:- Sometimes, fibroids remain undetected because woman experiences no pain and they don’t affect her daily life. In case, woman experiences heavy bleeding along with abdominal pain, medications and surgery are used for treatment. Medications:- • GnRHA- (gonadotropin released hormone agonist) is administered through injection to reduce the secretion of estrogen and to stop menstrual period. When estrogen levels are dropped, fibroids begin to shrink. • Tranexamic acid tablets are administered to enhance blood clotting in uterus. It reduces bleeding. • Contraceptive pills to stop menstruation. • Anti-inflammatory drugs like mefanamic and ibuprofen to reduce amount of prostaglandins associated with heavy periods. • LNG-IUS (Levonorgestrel intrauterine system) is a plastic device inserted into uterus. It releases levonorgestrel (progestogen hormone which reduces growth of uterus lining. As a result, bleeding is also reduced. Surgery:- • Hysterectomy - removing the uterus only in severe cases where woman experiences substantial bleeding or when fibroids are very large. • Myomectomy – to remove fibroids attached to the wall of uterus.. • Endometrial ablation – removing the uterine lining. • UAE (Uterine Artery Embolization) to stop blood flow to the fibroid. • Magnetic-resonance-guided percutaneous laser ablation – shrinking fibroids by using laser. • Magnetic-resonance-guided focused ultrasound surgery –shrinking fibroids by using sound waves. All of the treatment options have some side effects and they should be administered with proper discussion with physician.
