Rabeprazole with Domperidone belongs to a class of antisecretory compounds (substituted benzimidazole proton-pump inhibitors) that do not exhibit anticholinergic or histamine H2 -receptorantagonist properties, but suppress gastric acid secretion by inhibiting the gastric H+/K+ ATPase at the secretory surface of the gastric parietal cell. Because this enzyme is regarded as the acid (proton) pump within the parietal cell, rabeprazole has been characterized as a gastric proton pump inhibitor. Rabeprazole blocks the final step of gastric acid secretion. In gastric parietal cells, rabeprazole is protonated, accumulates, and is transformed to an active sulfenamide. Domperidone is a derivative of benzimidazole that possesses both prokinetic and antiemetic properties due to its inhibitory action at dopamine D2 receptors. Domperidone is a dopamine antagonist with anti-emetic properties domperidone does not readily cross the blood-brain barrier. In domperidone users, especially in adults, extrapyramidal side effects are very rare, but domperidone promotes the release of prolactin from the pituitary. Its anti-emetic effect may be due to a combination of peripheral (gastrokinetic) effects and antagonism of dopamine receptors in the chemoreceptor trigger zone, which lies outside the blood-brain barrier in the area postrema. Rabeprazole sodium is chemically known as 2-[[[4-(3-methoxypropoxy)-3-methyl-2-pyridinyl]-methyl] sulfinyl]-1H-benzimidazole sodium salt. It is a proton pump inhibitor and used for the treatment of peptic ulcer or GERD. It is not official in any pharmacopoeia. Domperidone is chemically known as 5-chloro-1-[1-[3-(2, 3-dihydro-2-oxo-1H-benzimidazol-1-yl) propyl] piperidin-4-yl]-2, 3-dihydro-1H-benzimidazol-2-one. It is a gastro-kinetic and anti-emetic. It is a peripheral dopamine-2 receptor antagonist. It is official in B.P. The present work describes the development of a validated RP-HPLC method, which can quantify these components simultaneously from a combined dosage form. A few chromatographic methods have been described for the individual determination of rabeprazole and domperidone in both dosage forms and biological fluids1-5. Domperidone has the characteristic features of: • Selective CTZ blocker. • Anti-dopaminergic agent, competitively inhibits the D2 receptors. • Acts in two ways: - Prevents the messages from reaching the Emesis center in the brain. - They increase the gastric emptying rate, by stimulating the rate & force of intestinal contractions and clearing the region of the residual food. • Thus, it removes the cause of emesis. • It is also termed as ‘PROKINETIC.’ • It does not cross the blood brain barrier, hence shows lower incidences of extrapyramidal side effects. One capsule of rabeprazole and domperidone was swallowed in empty stomach each day for up to 4 weeks by the patients. Rabeprazole and domperidone provided significant and remarkable improvement in symptoms of GERD. Although, the improvement was observed at first follow-up visit, continuing treatment for 4 weeks provided additional gains. Almost all patients tolerated the drug well. Most patients (94%) had excellent or good relief as assessed by their physician whilst 86% of patients rated treatment with rabeprazole and domperidone as good or excellent. Rabeprazole and domperidone not only provided desired relief of symptoms of GERD but also is very well tolerated. This combination may also improve the quality of life of patients suffering from GERD.
