For older women with invasive breast cancer, treatment withbrachytherapy following a lumpectomy is associated with a decreasedlikelihood of long-term breast preservation and an increasedlikelihood of complications, but no difference in overall survival,compared to whole-breast irradiation treatment, according to astudy published in the May 2 issue of the Journal of the American Medical Association . TUESDAY, May 1 (HealthDay News) -- For older women with invasivebreast cancer, treatment with brachytherapy following a lumpectomyis associated with a decreased likelihood of long-term breastpreservation and an increased likelihood of complications, but nodifference in overall survival, compared to whole-breastirradiation (WBI) treatment, according to a study published in theMay 2 issue of the Journal of the American Medical Association . Grace L. Smith, M.D., Ph.D., M.P.H., from the University of TexasMD Anderson Cancer Center in Houston, and colleaguesretrospectively analyzed data from 92,735 women (aged ≥67years) with incident invasive breast cancer, diagnosed between 2003and 2007 and followed up through 2008. Patients were treated withbrachytherapy (6,952 women) or WBI (85,783 women) followinglumpectomy.
The researchers found that the five-year incidence of subsequentmastectomy was significantly higher in women treated withbrachytherapy compared to WBI (3.95 versus 2.18 percent), and thisassociation persisted after adjusting for other variables.Brachytherapy was associated with significantly more frequentinfectious and noninfectious postoperative complications (adjustedodds ratio, 1.76 and 2.03, respectively). Brachytherapy alsocorrelated with significantly higher five-year incidence of breastpain, fat necrosis, and rib fracture. Differences in five-yearoverall survival were nonsignificant for patients treated withbrachytherapy versus WBI (87.66 and 87.04 percent, respectively;adjusted hazard ratio, 0.94; P = 0.26). "Potential public health implications of these findings aresubstantial, given the high incidence of breast cancer, along withthe recent rapid increase in breast brachytherapy use," the authorsconclude.
"Although these results await validation in theprospective setting, they also prompt caution over widespreadapplication of breast brachytherapy outside the study setting."
