Calcitriol Injection is synthetically manufactured calcitriol and is available as a sterile, isotonic, clear, colorless to yellow, aqueous solution for intravenous injection. Calcitriol is available in 1 mL ampules. Each 1 mL contains: Calcitriol 1 mcg; Polysorbate 20, 4 mg; Sodium Chloride 1.5 mg; Sodium Ascorbate 10 mg added; Dibasic Sodium Phosphate, Anhydrous 7.6 mg; Monobasic Sodium Phosphate, Monohydrate 1.8 mg; Edetate Disodium, Dihydrate 1.1 mg added; Water for Injection q.s. pH range 6.7-7.7.Calcitriol is the active form of vitamin D3 (cholecalciferol).Vitamin D3 must be metabolically activated in the liver and the kidney before it is fully active on its target tissues. The initial transformation is catalyzed by a vitamin D3-25-hydroxylase enzyme present in the liver, and the product of this reaction is 25-(OH)D3 (calcifediol).Calcitriol is a man-made active form of vitamin D. Most people get enough vitamin D from exposure to the sun and from fortified food products (e.g., dairy products, vitamins). Vitamin D helps control parathyroid hormone and the levels of certain minerals (e.g., calcium, phosphorus) that are needed for building and keeping strong bones. Calcitriol is used in the treatment of low levels of calcium in the body of patients on chronic renal dialysis and patients with hypoparathyroidism. Calcitriol is also used to treat secondary hyperparathyroidism and resultant metabolic bone disease in patients with kidney failure who are not yet receiving dialysis.A non-aluminum phosphate-binding compound should be used to control serum phosphorus levels in patients undergoing dialysis. Overdosage of any form of vitamin D is dangerous.Progressive hypercalcemia due to overdosage of vitamin D and its metabolites may be so severe as to require emergency attention. Chronic hypercalcemia can lead to generalized vascular calcification, nephrocalcinosis and other soft-tissue calcification. Calcitriol tablets should be used with caution in: * People who previously have been taking a long-acting vitamin D preparation, such as ergocalciferol and colecalciferol * People who have a sudden increase in calcium in their diet, such as increasing the amounts of diary products eaten * People who are unable to move around a lot, such as those who have undergone surgery * Patients who are dehydrated * Patients with kidney failure * Women who are pregnant. Calcitriol is used to treat hyperparathyroidism (overactive parathyroid glands) and metabolic bone disease in people who have chronic kidney failure and are not receiving dialysis.Calcitriol is also used to treat calcium deficiency (hypocalcemia) and metabolic bone disease in people who are receiving dialysis.Calcitriol is also used to treat calcium deficiency in people with hypoparathyroidism (underactive parathyroid glands) caused by surgery, disease, or other conditions. There are no data on the excretion of calcitriol topical into human milk. Because many drugs are excreted into human milk, the manufacturer recommends caution if calcitriol topical is administered to a nursing woman.Calcitriol topical has been assigned to pregnancy category C by the FDA. Animal studies have shown calcitriol to be fetotoxic. Animal studies of topically applied calcitriol induced a significantly elevated mean postimplantation loss and an increased incidence of minor skeletal abnormalities due to retarded ossification of the pubic bones.
