Hyperacusis and misophonia- their causes and treatments

The human ear cannot tolerate high decibel sounds. They are not equipped to handle loud sounds. Sensitivity to noise is that we are born with. But if a person experiences discomfort after exposure to low decibel sound then it points towards auditory processing disorder. This reduced sound tolerance has been linked with hyperacusis and misophonia.

Hyperacusis:

A person affected with hyperacusis is unable to tolerate sound that is normal for an average listener. The sound that crosses 80 dB is considered as noise. For these patients, hearing a 40 dB sound can be a cause of great comfort. Normally people do not have issue listening to low decibel sounds but in this case, the person is extremely sensitive to low intensity sounds.

Patients suffering from this condition describe a low decibel sound as painfully loud. Even if you speak softly, the person will feel as if you are uttering something loudly.

Causes:

Though the causes are not identified it is believed that it can occur because of:

• Head injury • Damage to the central auditory system • Brief exposure to noise • Lyme disease • Brain infection • Meningitis • Prolonged use of earplugs or earmuffs

Misophonia:

This refers to aversion to certain sounds, which is not related to the level of decibel.In this case the person hates to hear a particular sound, not because he perceives it as loud, but he simply dislikes it, especially if it is repetitive. This condition is not related to the volume of the sound. The person is comfortable to hearing the everyday sound. It is just that particular patterns of sound appear offending. For example sounds related to laughing, coughing, yawning, snoring are extremely annoying for patients and make them angry. The decibel level of the sound in this case is very low, but yet it can be unpleasant to people suffering from misophonia. It is considered as incorrect processing of sound waves in the brain.

Treatment:

In both cases the patients experience negative emotions such as fear, anxiety and rage after getting exposed to the offending sound. These people tend to avoid social events and hence reduce their interaction which makes them susceptible to depression.

•Treatment for hyperacusis:

People affected with this may need to undergo tinnitus retraining therapy (TRT) to increase their sound tolerance. The purpose of this therapy is to increase the threshold of the sensitivity of sound. It is achieved by exposing the patient to wide range of sounds, accompanied by counseling from an audiologist. This combination of counseling and sound therapy frequently works to improve sound tolerance.

•Treatment for misophonia:

In this case the patient may require consultation with an audiologist as well as a psychologist. The audiologist directs a specific type of sound therapy and the psychologist conducts cognitive behavioral therapy (CBT).
