Dysmenorrhea is categorized into two types on the basis of causal agents. Primary Dysmenorrhea occurs on account of extreme uterine contraction by a chemical called prostaglandin during menses. As a result, blood vessels in the vicinity get chocked, depriving muscles of the oxygen. This inadequacy of oxygen causes pain. This disorder usually starts in adolescence when a girl begins to have her first period. The pain which may begin before, after or during period is usually located in limbs, abdominal region, and lower back. Accompanying symptoms of tenderness in breasts, nausea, vomiting, dizziness and even mild diarrhea and fever are observed in many women. Diagnosing tests:- Physical examination Blood test to check the amount of prostaglandin Laparoscopy to check abnormal tissue growth or fibrosis Imaging tests such as MRI or Ultra sound. Risk factors of Primary Dysmenorrhea:- Early menarche Retroverted (Backward tilted)uterus Narrow cervical canal Obesity Heredity Environmental Factors Depression/Stress Treatment for primary Dysmenorrhea:- For Short term;- Hot bath Using Hot water bottler or heating pad to reduce contraction of uterine muscles Abdominal massage Intake of aspirin or some painkiller Drinking warm beverages like tea, soup(No coffee) Resting as much as possible Listening to music/ Doing some favorite activity to divert attention Avoiding over-exertion For Long term:- Regular exercising Lifestyle changes- avoiding Caffeine, Alcohol, Smoking, Fast food Intake of nutritious food Use of contraceptives(Oral pills, IUD with medical advice) Use of non-steroidal anti inflammatory drugs e.g. Diclofenac, Ibuprofen, Ketoprofen etc. Hysterectomy in severe cases Relaxation techniques like yoga. Alternative medicinal practices like Acupuncture It is difficult to treat Primary Dysmenorrhea if underlying cause is not found. Thus, despite many medications, pain may persist. The intensity of pain may reduce with age and completely disappear after childbirth. Although often neglected, causes great discomfort and may lead to school or work absenteeism resulting into cognitive or monetary loss. Hence, proper attention should be paid to nutrition, exercising and overall healthy lifestyle to manage it successfully.
