Hemorrhoids are commonly referred to as piles while some other people call it different names like haemorrhoids, hemroids or figs. When external hemorrhoids are complicated by the formation of blood clots they will become thrombosed making the pile swelling very painful and grossly enlarged. Thrombosed external hemorrhoids appear as severely painful swellings that look like semi ripe black currants. They are usually situated on the lateral (side) region of the anal margin. Internal Hemorrhoids: Internal hemorrhoids are swollen inflamed veins situated inside the anal canal underneath the anal mucosa. Most times apart from causing occasional painless bleeding during defecation, these types of piles are not usually associated with much symptoms and quite a number of them resolve on their own without causing any symptom. Internal hemorrhoids usually became symptomatic when they develop complications. The 2 common complications of internal hemorrhoids are:(1) prolapsed (2) strangulated hemorrhoids. Prolapsed hemorrhoids: Prolapsed hemorrhoids occur when the internal piles becomes so big or swollen that they protrude out of the anus. Initially the protrusion is slight and occurs only during defecation and this level of protrusion also reduces spontaneously. Later these prolapsed hemorrhoids do not reduce spontaneously but have to be reduced manually. They are known at this stage as third degree hemorrhoids. When the piles became permanently prolapsed they are known as fourth degree hemorrhoids. The prolapsed hemorrhoids can become gripped by the external sphincter causing further impairment of blood flow out of the swelling and worsening the congestion and pooling of blood in the already dilated veins leading to a complication called strangulation. Strangulation is accompanied by severe sudden pain and the victim refers to it as an acute attack of piles. What causes Hemorrhoids? 1. Hereditary factors: Hemorrhoids are frequently seen in members of the same family so there must be predisposing factors such as congenital weakness of the vein walls or an abnormally large arterial supply to the rectal blood vessels that is common to all the members of such families. 2. Excessive straining during defecation accompanying constipation or induced by diarrhea is a strong underlying factor to developing hemorrhoids. 3. Poor eating habits characterized by consumption of more of low fibers processed foods also contributes to development of hemorrhoids. 4. Pregnancy: The state of pregnancy predisposes to hemorrhoids. 5. Dehydration: Lack of proper hydration caused either by not taking enough water or excessive water loss makes the stools hard and the alimentary system more prone to constipation. 6. Sometimes hemorrhoids appear as result of other diseases like cancer of the rectum which compresses the rectal veins to prevent proper drainage of blood out of the anal and rectal veins leading to congestion and excessive pooling of blood. 7.
